New Cancer Support team

The Cancer Support team is a brand-new exciting service which forms part of the Well Up North Primary Care Network (PCN). The service is being piloted initially in AMG before rolling out across Well Up North PCN and will allow all cancer patients, their families, and friends to gain access to the support they need in such a difficult period of their lives.

We are here to offer a wide range of support services depending on the needs of the patient, and we can offer a face to face, home visit or telephone appointment. We are easily accessible via your GP surgery, simply ring and ask to speak to us or you can self-refer via our Well Up North App. Access the app using the QR code attached to our poster.
